The Healthy Smiles Plan is a savings plan membership for patients without traditional dental insurance who want an affordable way to prioritize their oral health. This is not dental insurance. This is an in-office membership provided by Oak Grove Dental Co. created for patients to help make quality dental care accessible and affordable.
Please Note: Additional fees may apply for perio maintenance or other treatment that is not considered a routine preventive cleaning.

The Healthy Smile Plan includes:
2 Dental Cleanings per year -OR- 2 Perio Maintenance per year

All radiography (2D x-rays)
2 Fluoride Treatments
Annual Cancer Screening
Free Consultation
20% discount on all other services
Annual Membership Rates:
Individual: $359.00
Couple (2): $645.00
Family (4): $865.00
* Children must be under 26 and currently living at homeEach Additional Person: $218.00
Perio Plus: For 3-4 month recall patients (at $150 to each Membership Rate)
Requirements:
Membership is valid for 12 months from the effective date.
Membership fees are non-refundable.
Membership benefits are non-transferable.
Membership discounts cannot be combined with dental insurance or other promotional offers.
All payments are due at the time services are rendered to receive membership pricing.
Patients using third-party financing (Cherry) receive a 15% Healthy Smiles discount instead of the standard 20% member discount because of financing merchant fees.
I understand membership benefits cannot be applied to treatment completed before enrollment.
No refunds on membership fee/premiums will be issued at any time if participant decides not to utilize the Healthy Smiles Plan.
I understand some services may not be able to be completed in office. Referrals to an oral surgeon, periodontist, and endodontist are not covered under this plan.
